The Collaborator Access Request Form is for account creation for users that need access to data and/or systems hosted by SMPH IIT. The data custodian (owner of data) or Division Administrator authorizes the access. If the data or system is part of an IRB protocol, the IRB protocol should be attached to the request and include the authorized users. UW-Madison HIPAA Privacy and Security Training is required by all members of the UW HCC prior to accessing PHI and annually thereafter, and also annually by all members of the School of Medicine and Public Health. https://compliance.wisc.edu/hipaa/training/
