DOM IT Purchasing Request
This form is used to submit technology purchasing requests to the DOM Helpdesk. It also allows the appropriate accountant to review and approve funding and for orders to be tracked by Divisions.
Submit DOM IT Purchasing Request
UW Health employed APPs need to request technology purchases through UW Health: https://uwhealth.service-now.com/sp - must access this link via onsite network or vpn.
*Please Note*
- All items ordered with University funding are owned by the University (i.e. federal grants, startup, gifts, and UWMF faculty professional funds). Under no circumstances do the items ordered become the property of the faculty and staff.
- If you are ordering the same items for multiple people, you only need to submit only one form.
- If you are ordering multiple items for one person, you only need to submit one form.
Snapshot of how this new process works
1. Submit your order request with funding and receive an email from Zendesk with your request or ticket number and the subject you entered in the details of your request.
2. If DOM IT has questions or would like you to review a quote, we will respond by email to this ticket number. If you have further questions you may also reply to the email you received.
3. Based on the funding you used, your request will be routed to the appropriate accountant for review and approval.
4. After the order is approved, DOM IT will place the order and you will receive a second email notifying you (the submitter) and the the Recipient/Requestor that the order was placed.
Tips for Filling out the form
1. Your email address required to submit the request
2. Recipient / Requester Name - only needs to be filled in if different than person submitting request. This is ultimately who the equipment will go to and whose name will be on the ticket (The Submitter Name will also receive a notification email when the order is placed.)
3. Please Specify Type of Item and Quantity
For Example: 1 Laptop, 2 Monitors, 3 Headsets, 4 Webcams
4. Subject - this will be the subject of the email you receive
5. Please give us a brief description - this is where you can add an additional details to your request

PLEASE NOTE: If you are ordering for CHDR, CHGPM, Emergency Medicine, OCT, Pediatrics, WAI & UWCCC, please select Non DOM Department/Center/Institute as funding type
